2016 Community Health
Improvement Plan
The Community Health Improvement Plan (CHIP) partnership is an innovative collaboration between Jefferson Healthcare (JCH), the City of Port Townsend and Jefferson County Public Health (JCPH) devoted to identifying the most pressing health priorities for Jefferson County and actions that will lead to improvements.
The CHIP partnership focuses on four Jefferson County health priorities, which were identified through a rigorous community data assessment study in 2014 and prioritization process meetings. The resulting CHIP document was completed in 2016. The four health priorities focused on include:
