CHIP's 2021 -'22 Update Effort 

CHIP 2021-22 Update - Overview 

In 2019 CHIP led a Community Health Assessment (CHA) process throughout Jefferson County.  From 2020-2021 community stakeholders have focused to consider the CHA data results, develop goals, and articulate goals, strategies, action and metrics for CHIP's 2021 Community Health Improvement Plan (CHIP) Update,   Agencies, organizations and community advocates, led by Lori Fleming, developed a draft update CHIP Plan.  The draft Plan will next be approved by the Governing Body in 2023..  Once the 2023 CHIP Update has been approved, the three Age-Band workgroups will begin to execute the Strategic Plan held therein.

CHIP's Development 2016 -2019

The CHIP partnership is an innovative collaboration between Jefferson Healthcare (JCH), the City of Port Townsend and Jefferson County Public Health (JCPH) devoted to identifying the most pressing health priorities for Jefferson County and actions that will lead to improvements.

The 2016-2019 CHIP partnership focused on four Jefferson County health priorities, which were identified through a rigorous community data assessment study and ranked at data assessment meetings. The four health priorities focused on include:

  • Access to care

  • Access to mental health and substance abuse services

  • Healthy eating, active living and chronic disease prevention

  • Immunizations


CHIP was awarded a $1,000,000 federal grant in the Fall of 2020 to continue the work of the Behavioral Health Consortium (BHC) through August 2023.,  The BHC's Strategic Plan was developed with funds from a planning grant awarded in 2019.  The focus of these efforts are on Opioid and Behavioral Health Prevention, Treatment, and Recovery.  This work addresses a key priority identified in the CHIP's 2016 Strategic Plan and is funded a Rural Communities Opioid Response Program (RCORP-I) grant, awarded by HRSA, the federal Health Resources and Services Administration. 2019 Community Health Assessment

 The 2019 Community Health Assessment (CHA) was conducted in Jefferson County.  Qualitative input was gathered from the community through Community Forums and Conversations and Key Informant interviews.  The quantitative data on specific measures was sourced from various existing data resources and a community survey.  We invite you to click through to the CHIP website to peruse related CHA documents and presentations.
